Health Information for your student needs to be updated in Skyward every school year by the parent/guardian. This can be done by following this (provide link) tutorial. Please keep these (provide link for Skyward Health info tips) tips in mind as you complete this information. If you have questions, you can contact the IPS District Nurse for assistance.

The following forms will also need to be completed (if applicable) for your student and turned into the secretary at their school on or before the first day of school. You can do this through the fillable online form below (click on the blue link), print the documents below, or you can obtain this form from your student’s school.
